The FourLeaf Charitable Giving Program is committed to strengthening the quality of life in the communities we serve. We ask any non-profit organization who receives support from FourLeaf to complete an Outcomes Assessment Report. Your feedback will provide the necessary information to track the impact and the dollars we invest in the Long Island community.
Organizations who received support from FourLeaf Federal Credit Union’s Charitable Giving program within the last three years must complete the Outcomes Assessment Report before applying for new or renewed support. We would like the Outcomes Assessment Report to be submitted to us at the completion of the grant timeline or sponsorship event.
